Medical Records

Daily Activity Logs or Function Reports

You or someone close to you may need to fill out forms describing how your disability affects your ability to perform everyday activities—like cooking, bathing, or shopping.


SSA-Specific Forms

  • SSA-827 (Authorization to Disclose Information)
